Document Exception Review: Decision and Handoff

A review workflow makes uncertain or rule-failing output visible to the person authorised to decide what happens next.

Define the review trigger

Examples include missing mandatory identifiers, ambiguous dates, incomplete tables and results outside approved specifications. Separate extraction errors from genuine business exceptions.

The trigger should explain what needs checking. It should not silently approve a record because processing finished.

Give the reviewer the right evidence

Retain the permitted source, relevant output, template revision and reason for review. Use an authorised reference system when the document alone is insufficient.

Record who reviewed the result, the decision, the reason and any correction according to the approved workflow. Acknowledgement is not correction.

Use explicit outcomes

OutcomeMeaning
AcceptRequired checks are satisfied under the approved process.
CorrectA source-supported correction is recorded and revalidated.
RejectThe result must not proceed as accepted output.
EscalateAnother authorised person or source is needed.

Keep downstream actions separate

Approving an extraction does not automatically authorise payment, a stock release, an ERP amendment or another operational action. Confirm the receiving process and its approval boundary.

Measure time to reviewed output and unresolved exception age rather than only counting completed jobs.
